In general the Medicine Seller should be able to access most of the basic tools of their trade without major Notoriety expenditure - the box itself would be considered a Magic Weapon in terms of Notoriety cost for its holding function and we may charge a small extra amount for the general reagents carried but most of that isn't particularly hard to come by.
Unique items will of course be considered Magic items based on the number of effects they carry which will determine how much the regain is.
The sword could certainly count as a familiar if it is sentient/conversant with the Medicine Seller, that would be fine.
